29 CFR 1910.147(c)(4)(i):Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: On or about February 17, 2022, set up technician employees engaged in mold and tooling set up activities were exposed to caught-in and/or caught-between hazard in that the specific machine procedures for a complete program rod removal task from the Davis Standard Sterling Extrusion Molding machine #9, Twin 15lb head S#07834 which had multiple energy sources including pneumatic, electrical, hydraulic, and gravity were not developed, documented and utilized.
