29 CFR 1910.307(c)(3):Equipment was not safe for the location in that it was not of a type and design that the employer demonstrated would provide protection from the hazards arising from the combustibility and flammability of vapors, liquids, gases, dusts, or fibers involved: On or about January 21, 2022, employees in the mixing room in Building Q, which is a Class I, Division 2 classified area were in the process of cleaning dust off of Mixing Tank 2. The employees used a Shop-Vac vacuum and two extension cords which were not rated as safe for the location. Employees were exposed to fire and explosion hazards when flammable vapors were ignited causing a flash fire and an explosion in Mixing Tank 2. Abatement documentation is required of this item in accordance with the requirements of 29 CFR 1903.19(d).
