1926.20(b)(1) The employer did not initiate and maintain accident prevention programs as necessary to comply with this part. a) Jobsite, 10507-10515 Clapp Simms Dude Road: On or about January 5, 2022,an excavator was digging dirt to install a sewer pipe inside a 5 foot deep trench while an employee moving dirt with a shovel directly underneath the excavator bucket . The employee was struck-by the excavator bucket, resulting in an amputation injury to their hand.
