Walnut St. Bucyrus, OH: The employer did not ensure the power transmitting belt and pulley(s) of the direct drive motor for the "Bulb crusher" HS1 coater conveyer was adequately guarded. Following a maintenance and cleaning operation, an employee working on or near this unguarded nip-point was exposed to amputation hazards.
