29 CFR 1904.39(a)(2): The employer did not report the in-patient hospitalization of one or more employees to OSHA within twenty-four (24) hours. a. On July 15, 2021, at Rocktenn Corrugated Packaging, 53 Industrial Parkway, Syracuse, New York, an employee was knocked to the ground after being struck by a tractor trailer door. The employee sustained a fractured vertebra and was hospitalized on that day. OSHA was not notified of the hospitalization until October 8, 2021.
