29 CFR 1910.22(d)(1): The employer did not ensure that walking-working surfaces are inspected, regularly and as necessary, and maintained in a safe condition: Employees are exposed to struck-by hazards. Employees operating power industrial trucks in tandem to move baghouses were exposed to a broken and uneven floor exiting the production area. The uneven floor condition caused the load to become unstable resulting in the amputation injury.
