29 CFR 1910.135(a)(1): The employer did not ensure that each affected employee wear a protective helmet when working in areas where there is a potential for injury to the head from falling objects: On or about July 8, 2021, employees were exposed to a struck by hazard while performing tree removal activities using a Bandit Model 250 Wood Chipper. An employee was injured while the foreman was unjamming a clog in the wheel chamber and the employer did not ensure that the employees was wearing protective helmet.
