(i) Except as provided elsewhere in this section, the employer must ensure that each employee on a walking-working surface with an unprotected side or edge that is 4 feet (1.2 m) or more above a lower level is protected from falling by one or more of the following: On or about May 11, 2021, the employer did not ensure an employee working inside the Hot Strip Mill #2 Reheat Furnace was protected from falling approximately 19 feet from the furnace pre-heat zone walking-working surface into the furnace scale pit, by a guardrail systems, safety net system, and/or personal fall protection system.
