29 CFR 1910.212(a)(1): Machine guarding was not provided to protect operator(s) and other employees from hazard(s) created by: (a)P-Line Extruder; On or about April 13, 2021, employees worked on and around the feeder transition piece of the extruder. The butterfly valve near the end of the feeder transition piece was not properly guarded and allowed employees to come in contact with the valve when dislodging the polytubing from the inside of the transition piece.
