29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: (a) Production - On March 18, 2021, the employer did not ensure that removal methods or other controls were used to eliminate employee exposure(s) to a pinch point area created between the framework for Machine #805 and the reel during the removal of the reel at the take-off end. On March 18, 2021, an employee suffered a finger amputation while attempting to remove the drive pin during manual reel replacement. No abatement certification or documentation is required for this item.
