29 CFR 1910.23(b)(9): The employer did not ensure that ladders are inspected before initial use in each work shift, and more frequently as necessary, to identify any visible defects that could cause employee injury. On or about 2/1/2021, at the Lower Poplar Wastewater Treatment Facility, 1101 Lower Poplar St. Macon, GA 31201: Employees using an extension ladder were exposed to struck by and amputation hazards; in that the employer failed to identify the defects by performing an inspection before use.
