1926.21(b)(2) The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury: a) YMCA Gymnasium, Fairless Hills, PA - On or about January 29, 2021, employee(s) were operating a JLG Scissor lift, with the platform elevated approximately 20-25 feet above the ground level. The scissor lift was utilized as material support and lifting device for ductwork removal which created stability issues and exposed employee(s) to fall hazards.
