29 CFR OSH ACT of 1970 Section (5)(a)(1):The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees from entanglement hazards: Professional Service Industries, Inc. Dba Intertek-PSI, at the work-site located at 32 Willow Lane, Oostburg, WI: On or about January 6, 2021 employees utilizing a Diedrich D-50, vehicle mounted drilling rig were not protected from entanglement hazards from the rotating hollow stem auger.
