29 CFR 1910.147(a)(3)(i): The employer failed to establish a program and utilize procedures for affixing appropriate lockout devices or tagout devices to energy isolating devices and to otherwise disable machines or equipment to prevent unexpected energization, startup or release of stored energy in order to prevent injury to employees. On or about September 30, 2020 and times prior thereto, the employer did not establish procedures so authorized employees to lockout and tagout machines prior to performing maintenance exposing them to struck-by and caught-in hazards.
