OSH ACT of 1970 - Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to an employee, in that employees were exposed to amputations and other serious bodily injuries from inadequately secured mold plate displacing and falling during maintenance operations work. Location: Mold Tool Room, 10 Leighton Road, Milford, CT 06460 On or about August 13, 2020, the employer did not ensure a mold plate measuring approximately 2.5 inches thick, 34.5 inches long and 36 inches wide, with a weight of approximately 650 pounds, was sufficiently secured during maintenance operations resulting in the mold plate shifting (moving uncontrolled) while being repositioned manually and falling amputating the right fingertip of a worker.
