Text For Citation: 01 Item/Group: 001 Hazard:
29 CFR 1910.147(d)(2): The machine or equipment was not turned off or shut down using the procedures established for the machine or equipment: (a) Facility, 311 Drum Avenue, Somerset, PA: On or about June 29, 2020 the bakery line B was not turned off or shut down when an employee attempted to replace a spring on a cone forming tooling unit. The employee opened the cover and reached into the unit to replace the spring when the machine cycled and caught his left ring and pinky fingers in the press. The injury resulted in the amputation of the left pinky finger.
