29 CFR 1904.39(a)(2): The employer did not report an in-patient hospitalization, amputation, or loss of an eye as a result of a work-related incident to OSHA within twenty-four (24) hours: a. On or about 2:25 PM on March 1st 2019 at the Energy Building project Saulsalito, Lake Buena Vista, FL 33830, an employee suffered a work related amputation on his left hand index finger. The employer was aware of the amputation on March 1st 2019. The employer notified OSHA on March 4th at 8:00 AM of the work related amputation.
