29 CFR 1910.147(d)(4)(i): Lockout or tagout devices were not affixed to each energy isolating device by authorized employees: On or about January 4, 2019 at 12000 Lawndale St, Houston TX 77017, the employer did not affix Lockout and Tagout devices as required by company procedures: a) On or about January 4, 2019, the employer did not affix a lockout device to the closed valve on the abandoned former Magellan pipe. This condition exposed employees to flash fire hazards and struck-by hazards.
