Text For Citation: 01 Item/Group: 001 Hazard:
OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment that were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to struck-by and crushing hazards while uncrating insulated glass units: a) 1800 Arch St., Philadelphia, PA 19103 - The employer did not protect employees from struck-by and crushing hazards while uncrating insulated glass units. This condition was noted on or about September 29, 2018, when an employee was seriously injured when replacement glass units weighing approximately 5,500 lbs. became unstable during the uncrating process and fell onto the employee. The Enclos Corp. was previously issued a Hazard Alert Letter on October 25, 2016 for an incident which was contained in OSHA inspection number 1149542, with respect to a workplace located at 1800 Arch St, Philadelphia, PA. Abatement note: Among other methods, one feasible and acceptable means of abatement would be to ensure that the company enforces its own Activity Hazard Analysis for uncrating insulated glass units. The employer will ensure that during the uncrating process glass units are secured from tipping and falling with rigging equipment.
