29 CFR 1910.212(a)(2): One or more methods of machine guarding were not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. The guard affixed to the machine offered an accident hazard in itself: a) In the Warehouse; on or about January 6, 2017 - The safety gate affixed to the baler machine created a pinch point where a caught between hazard existed on the left side of the safety gate and latch. An employee operating the baler, amputated their pinky finger while pulling the rope attached to the handle of the safety gate closed on the baler.
