Text For Citation: 01 Item/Group: 001 Hazard:
Section 5(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to fire and/or explosion hazards: (a) On or about September 14, 2011, Kline 5300-11-18H well site near Alexander, North Dakota, approximately four employees were exposed to fire and/or explosion hazards on a service rig where the BOP (blowout preventer) was not installed properly and/or tested. Two employees were killed and two others suffered severe burns; one with both legs amputated. Among other means, one feasible and acceptable means of abatement would be for the employer to ensure all BOPs are installed and tested in accordance with best established industry practices, the manufacturer's guidance, and references established by the IADC (International Association of Drilling Contractors) and API (American Petroleum Institute) under its Recommended Practice 54 Occupational Safety for Oil and Gas Well Drilling and Servicing Operations.
